Using the templates

  1. Download, fill or print

Type and tick boxes in the PDF on any device, or print on letter paper. The SST, toolbox and violation forms are wide, so print them sideways.

  2. Fill in the project header

Address, BIN, permit holder and the person responsible for the log that day.

  3. Record as you go

Sign and date every entry. On a major building, inspections go into the site safety log by the end of the day.

  4. Keep it on site

Logs must be at the job site when the Department asks for them.

What the NYC Building Code requires

Building Code Chapter 33 and the DOB rules ask for written records of safe practice on site. At major buildings, the Site Safety Manager or Coordinator keeps a site safety log of every inspection, meeting, unsafe condition, correction and violation. Inspections are recorded by the end of the day, and the construction superintendent reviews the previous day's entries and signs the log before the next day's work starts (BC §3310.8.4). On jobs with a construction superintendent and no Site Safety Manager or Coordinator, the superintendent inspects the work areas on each visit and completes a signed log before leaving the site (BC §3301.13.13, 1 RCNY §3301-04).

Equipment has its own paper trail. A supported scaffold is inspected before each shift by the competent person supervising its use, and the result goes into a signed and dated pre-shift report (BC §3314.4.3). Sidewalk sheds need a written inspection report at installation and every six months after that (BC §3307.6.5). Crane and derrick operations need pre-shift and post-shift inspection records, pre-shift meeting entries and custody documents kept at the job site (1 RCNY §§3319-01, 3319-02). Where the site safety training law applies, the permit holder also keeps a daily log of every worker, with a copy of each person's SST card (BC §3321). Standpipe systems stay in a state of readiness, and where a Site Safety Manager or Coordinator is required, every alarm activation, inspection and repair goes into their log (BC §3303.8).

A missing or incomplete log can draw a violation, and it is easy to prevent. These templates give your field team one format for each record. They are a free resource, they are not official DOB forms, and they do not replace the code. Check the current requirements with the NYC DOB and the FDNY.

Need more than a template?

A log is one part of a compliant site. If you face a site safety plan requirement, a superintendent designation, an open violation or a stalled permit, we prepare the filings and the plans and follow the job through to sign-off.

Questions about what your site must keep on record?

Send the address and what you plan to do. We tell you which permits and filings the work needs and what we would prepare. The review is free.
